State Plan Amendment

A State Plan Amendment (SPA) is the formal process states use to modify their Medicaid state plans. When states want to change how they pay healthcare providers, what services they cover, or their reimbursement rates, they must submit a SPA to the Centers for Medicare & Medicaid Services (CMS) for approval.
